Thank you for your interest in volunteering with Foodwise! Please complete the following application form. 

In addition to completing this form, all volunteers are required to attend one of our volunteer orientations; check our Volunteer page for upcoming dates and to sign up. If you are not able to attend or would like to start volunteering before our next orientation, please complete our online application below and email to request a link to an orientation recording.

Once you have completed your application and orientation, we’ll start sharing volunteer opportunities with you so you can start volunteering. 

Below, you’ll be asked to accept the terms of the following documents. Please click each one to review (there is no need to sign the PDFs; please confirm your acknowledgment in the form below):

• Volunteer Handbook (required)
• Nondisclosure Agreement (required)
• Waiver and Release of Liability and Hold Harmless Agreement (required)
• Photo Release (optional but recommended)
